General Duties
- Work in partnership with the SBP team.
- Work collaboratively with professionals from respective social work organizations, the MSF, educational institutions, hospitals and any other organizations involved in the care plan of every child.
- To participate in the Children In Care Standing Committee (CICSC), Admission, Progress and Discharge Workgroup (APDW) and Care Plan Meetings, as required.
- Any other admin tasks assigned by Reporting Officer
Case Management
- Provide counselling support to assigned residents by regularly meeting them at least once a month and ensure that these sessions are maintained in written records in a timely manner.
- Assess residents’ overall wellbeing and formulate/implement a care plan to monitor the progress and development of the resident.
a. Recommend appropriate therapeutic intervention as part of care plan
b. Monitor and record changes in resident’s behavioural performance in care plan and provide updates during care plan meetings or when necessary.
c. To develop and enhance systems to improve existing services
d. Work with the Residential Care staff to ensure the implementation of care plan interventions.
e. To regularly evaluate the outcome of care plans and any other intervention carried out for the residents.
- Maintain proper record of all the services rendered to the resident in accordance to organization policy and MSF licensing requirement.
Family Engagement
- Implement permanency plans for residents who are unable to be reunified to identified carer or who had unsuccessful attempts of reunification back to carer.
- Maintain and update case recordings regularly.· Counsel residents and family members/significant adult to facilitate implementation of care plan and permanency plan.
- Work with various stakeholders & community partners (e.g. MSF & FSC) to plan and work towards family reunification/permanency planning.
Resident Management
- Co-lead with Residential Care staff to ensure the resident’s safety, health & positive behaviour when they are in the Home and during outings & appointments.
a. Monitor the residents to ensure that they are present in SBP, house or the activity you are supervising (or their whereabouts are known to you when they are not at the Home) and double check, communicate and give permission for residents to leave areas or activities under your supervision when it is requested or required.
b. Ensure that they follow and adhere to the structure in SBP.
c. Monitor the residents to ensure that they are physically safe and do not partake in any risky behaviour or activities which may cause themselves or others physical injury – especially during playtime.
d. Monitor & report to the Management of SBP of any complaints from residents, family members, volunteers or visitors to the Home.
e. Monitor & report to the Management of SBP of any signs or suspicions of physical, sexual or emotional abuse or self-harm.
f. Monitor & manage the behaviour of the residents by encouraging & rewarding good behaviour and by preventing, discouraging or discipline misbehaviour in strict adherence to the rules & discipline procedures of the Home.
g. Supervise residents and teach rules and provide reminders about safe touch & unsafe touch; and check in with residents to flag any behavioural concerns and issues.
Requirements
- Degree in Social Work
- 2 years’ experience in a social service setting. Fresh Graduates will be considered too.
- Ability to execute case management with minimal supervision.
- Knowledgeable and sound in social work theories and practices
- Agile and flexibility in working with children aged between 7 and 21
- Good team player with a high level of initiative and independence.
- Excellent interpersonal communication skills.
