Role Overview: As a Medical Social Worker at Ihsan Kidney Care, you will be responsible for delivering comprehensive care to patients in both the dialysis center and home dialysis settings. You will conduct assessments using tools such as the Bio-Psychological-Spiritual (BPSS) assessment and the Risk & Needs Matrix to guide patient management effectively. Your expertise in medical aspects and community dialysis settings will enable you to provide vital support to patients and their families.
Key Responsibilities:
- Engage with clients and conduct interviews to assess cases using various assessment tools, including BPSS and Risk & Needs Matrix.
- Develop holistic care plans to enhance patients' psychosocial functioning.
- Collaborate with caregivers to optimize care modalities and recommend respite when necessary.
- Administer a Job Placement Pilot Program tailored to IKC patients undergoing long-term dialysis.
- Conduct supportive counseling visits to dialysis centers, patients' homes, or hospitals.
- Advocate for patient dignity, confidentiality, and rights.
- Collaborate professionally with internal and external healthcare professionals.
- Network with government agencies, community organizations, and individuals to mobilize social resources.
- Facilitate support group programs for patients and caregivers.
- Maintain accurate records, including bio-data, session recordings, assessments, social reports, and patient feedback forms.
Requirements:
- Possess a Master's Degree, Degree or Graduate Diploma in Social Work.
- Minimum of 3 years of social work experience.
- Registered Social Worker (RSW) with the Singapore Association of Social Workers or eligible for RSW status.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint).
- Ability to speak local dialects or Malay/Mandarin/Tamil strongly preferred.
- Comfortable working in a fast-paced, multi-disciplinary team environment.
- Strong knowledge of the health and social sectors in Singapore.
- Excellent communication and interpersonal skills.
- Capable of working independently and collaboratively.
- Experience in casework, counseling, social and financial assessment, care plan development, and liaison with government agencies.
- Competent in means testing and other relevant returns submission.
- Skilled in managing staff of all ages.
