Job Descriptions:
- Implement nursing interventions and procedures based on the client's care plan.
- Provide specialized nursing services, delegating basic care to trained staff (Nursing Aides/Community Care Assoc/Healthcare Asst).
- Coordinate with the care team and update them on changes in the client’s condition.
- Prepare clinical documentation, monitor service outcomes, and escalate cases requiring acute management.
- Support and educate caregivers, clients, families, and medical practitioners.
- Supervise nursing aides and other care staff to ensure compliance with MOH guidelines.
- Facilitate activities to improve client care quality and support care integration.
- Conduct regular competency assessments for care staff, including hand hygiene, PPE, and medication assistance.
- Assist with incident reports related to client care.
- Provide monthly updates to senior nursing staff and assist with coverage as needed in other facilities.
Requirements:
- Recognized qualification in Diploma/Degree in Nursing
- A minimum of 5 years’ experience in clinical area such as acute or community hospital.
