As a Staff Nurse, you will play a central role in providing a high standard of nursing care to residents and their family members.
DUTIES & RESPONSIBILITIES
- Complies with approved standards of nursing practices
- Responsible for assessing the needs and delivery of quality geriatric / dementia nursing care to clients
- Perform pre-assessment of new clients refer by AIC
- Ensure the clients’ enrolment complies with the Company’s SOP
- Administer the documentation to be submitted to relevant government body
- Consult and coordinate with health care team to assess, plan, implement, and evaluate client’s conditions.
- Monitor, assess and review clients’ progression and implement holistic care plan
- Provide health promotion, counselling to clients and families
- Perform nursing care such as medication administration and wound dressing and other technical skills as required
- Oversee and manage the performances of the nursing team / care staff to ensure desired level of work competency
- Coordinate, implement, and monitor clinical and service quality performance indicators for continuous improvement plan
- Perform BCLS, CPR, AED and first aid to clients and plays an active role in emergency situations.
- Assist and support delivery of health promotion activities and In-service Education programs in the Centre.
- Participates in clinical service projects, infection control campaign activities.
- Participates in training and development through clinical guidance and direction to junior nurses.
- Utilize clinical judgement, critical thinking and evidence-based care (research based) in the delivery of resident care
- Coordinate continuity of care throughout hospitalization and transition to home/community
- Act as a liaison between residents, care givers and multi-discipline healthcare professionals and communicate effectively on the care plan and discharge planning
- Demonstrate accountability for one’s development and status
- Assume other duties and responsibilities as assigned by SNM / Centre in Charge
- Bedside handling and ward experience
- Coaching nursing experiences for Nurse Aides, Healthcare staffs (Care Assistants)
- Communicate and enforce Nursing & Senior Day Care standards, policies, and procedures.
- Lead infection prevention and control initiatives, including training, audits, and outbreak management.
- Update and review policies, ensuring they align with current needs.
- Develop staff competencies and address skill gaps for new and existing employees.
- Identify and train key competencies for Support Care Staff.
- Oversee standards and competencies of Staff Nurses.
- Collaborate with various departments and allied services for staff training.
- Conduct orientation programs for new staff.
- Ensure compliance with MOH, ENHS, and Clinical standards.
You will be responsible for:

- Evaluation of nursing care to meet the needs of the clients
- Attends to all emergency situation
- Update SOP (clinical)
- Administer / Witness in serving medications
- N95 Mask Fitting for healthcare staffs
- Conduct staff competency
- Conduct healthy courses for clients
- Order Medical Items and Disinfection Cleaning Items, maintenance medical stocks in the centre; checking for expire date.
- Prepare documents, client’s case files, SOP, training courses.
- Prepare documentations for upgrade current Hannah MDC Centre and PE@PP MDC Centre to Hannah EDDC Centre and PE@PP EDDC Centre.
- Prepare for MOH Audit – Renewal Two Years Maintenance Day Care Operations Licenses.
Other duties include preparation and assisting in clinical task, documentation, records co-ordination and updates on changes in residents' conditions and needs as well as providing cost-effective care in accordance to established policies and procedures, standard of nursing care and practices.
Qualifications
- Diploma / Bachelor's Degree in Nursing or Certificate in Gerontology
- Registered Nurse with the Singapore Nursing Board
- Valid BCLS + AED Certification
- 2 – 3 years of experience
- Advanced / Specialist Diploma in Geriatric / Community Nursing an added advantage.
- Experience in Nursing Homes / Geriatric wards / Hospitals are an added advantage.
- Open to travelling to different centres if required.
