RESPONSIBILITIES
MINDS Clinic is a service provided under the umbrella of MINDS Community-Based Support Services (CSS). CSS is the dynamic and nimble service delivery arm in MINDS that response to emerging needs of PWIDs and Caregivers in the community. Its core vision is to serve the unserved and underserved and to enhance integration and community living of caregivers and PWIDs in the communities.
Developmental Disability Nursing at MINDS Clinic play a pivotal role in the care of individuals with intellectual and developmental disabilities of all ages to achieve optimal health outcomes. The key role of the Enrolled Nurse is to focus on the promotion of health and wellness, prevention of illness, and care for clients in the community. He/She will reports to the Senior Manager, Health Services and will work alongside with the Clinic Doctors to provide support to both clinic staff and clients.
He/she will contribute towards achieving MINDS Clinic Vision's to be the Developmental and Intellectual Disability Clinic of Excellence, bridging prevailing social medical gaps so that PWIDS can receive effective healthcare management throughout their lifespan.
Clerical and Administrative Support:
- Attend to clients courteously upholding quality service standard.
- Support front-desk duties such as scheduling appointments, billing and payment collection, handle enquiries(email/phone/walk-in) as required.
- Registering clients' with their CHAS/HealtherSG/MediSave/insurance accounts.
- Chaperoning doctor/ Clients for patient examination in clinic & home visit.
- To coordinate staff meetings, hub activities as required.
- Design and develop health services collaterals as assigned.
- Support the Senior Manager, Health Services in proper use, care of medical equipments.
- Management of Clients' account and medical records.
Client Care Management:
- Adhere to infection control and prevention practices in client care activities.
- Support transitional care plans.
- Support and feedback clients social, medical needs during case management conference with allied health professional, registered nurse and doctors.
- Assist in procedures and treatments.
- Co-ordinate and support clients that required initial Medical Escort services with various polyclinics, hospitals.
- Support clinical duties across MINDS Clinics.
Client/Caregiver Education:
- Teach clients, caregivers on technique of selfcare, e.g. packing of medicine, follow up with doctors' instructions, and oral care hygiene.
Programme Management & Development:
- Support Clinic Specialist programmes, e.g. health screenings, vaccination, dental programmes.
- Administer vaccination as per EN license, e.g. subcutaneous injection
- Support the Senior Manager, Health Services on outreach programmes, e.g. GP outreach, Community Health Nurses Network, etc.
NITEC in Nursing and Above
OTHER INFORMATION- Practising License issued by Singapore Nursing Board.
- Technically skilled and competent.
- Good interpersonal and communication skills.
- Computer literacy skills - MS Word and PowerPoint.
- Experience working in the social services or health sector preferred.
- Demonstrated ability to work effectively and efficiently and a team player.
- Certified in BCLS.
- Experience in Medisave/CHAS/insurance claims would be an advantage.
- Able to perform Phlebotomy would be an advantage.
