COMPANY DESCRIPTION

The Children and Youth Group (CYG) in The Salvation Army Singapore serves hundreds of vulnerable children and youth yearly. In line with our vision to provide a continuum of care to uplift those in need, our services span from preventive services, foster care and residential services to reunification support services for vulnerable children returning to their natural families after being removed due to safety concerns. RESPONSIBILITIES

In addition to caring for children and youths, Children and Youth Group - Gracehaven also runs an Interim Placement and Assessment Centre (IPAC) in the residential home. This temporary care environment will allow children under child protection to be in a safe place while assessment into their needs are undertaken to facilitate their placement in the most appropriate care environment.As a Caseworker/ Social Worker in IPAC, you are expected to maintain a caseload as assigned by senior staff members and provide individual casework weekly counselling, groupwork intervention plans including behavioural management of clients in care.

Job Responsibilities

  • Participate in social work practice across disciplines
  • Build rapport and professional relationships with clients and significant others
  • Conduct needs and intake assessments
  • Perform case and care planning and goal setting
  • Implement case and care plans and interventions
  • Review casework interventions for improvements
  • Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
  • Maintain case notes and documents
  • Prepare social work reports
  • Conduct environmental scans and community need assessments
  • Collaborate with other agencies to pool community resources
  • Implement programmes
  • Support evaluation of programmes
  • Support the implementation of standards of care, organisational health policies, procedures and programmes
  • Identify areas of improvement in models of care and service delivery based on evidence-based practices
  • Respond to crisis situations based on organisational standard operating procedures
  • Ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met during the client's stay in the IPAC
  • Effective use of therapeutic approaches and other intervention strategies to stabilize and regulate emotions and behaviours or clients
  • Provide regular updates and work with Ministry case worker and other relevant professionals to facilitate decision making on placement plan for clients
  • Complete all necessary documentation for all clients, including staff observation forms, discharge summaries and other ongoing records.
QUALIFICATIONS

Degree in Social Work from an accredited institution.

OTHER INFORMATION

Working Hours

Is a 5-day work week and the total work hours is 44 hours per week.

9 am to 6 pm - Monday to Friday (inclusive 1 hour lunch break).

Flexibility in working hours is required in this role.
