Job Summary

Assist the MSW in administrative and clinical duties.


Prinicipal Accountabilities

  • Assist the Medical Social Worker in carrying out administrative work, in the delivery of social services to patients and their families.
  • Be involved in the pre-admission work, such as taking on a new referral, liaising with the external agencies and internal team to facilitate a resident’s admission to the nursing home.
  • Provide pre-admission and financial counselling to incoming patients and their families.
  • Assist patients to apply for appropriate financial assistance schemes when needed (Medifund, Household Means Testing (Subvention), MFEC and other funding schemes).
  • Assist applications of AIC Long Term Schemes (Eldershield, Elderfund, PGDAS, IDAPE, Careshield Life, etc.)
  • Keep updated register and records of patients (electronic or written data).
  • Attend to enquiries on referrals, service and charges.
  • Maintain accurate and confidential records of assessments, care plan, documentations, interventions / progress, statistics on patients and referrals from various sources in a timely manner.
  • Provide secretarial support to the MSW including data entry and filing to support the delivery of services to patients and/or families.
  • Be involved in casework management of patients.


Other duties as instructed by the MSW

  • Coordinate home visits and field work under the supervision of MSW.
  • Carry out other duties as assigned from time to time.


Requirements

  • Minimum ‘O’ level, ‘A’ level or Diploma holders with 2 years of relevant healthcare industry experience
  • Proficient in Microsoft Office applications
  • Possess empathy and patience when dealing with residents, next-of-kin and staff
  • Good interpersonal and communication skills
  • Bilingual (ability to speak local languages / dialect will be an added advantage)
  • A good team player
  • Has good attention to details
  • Hard-working and open to learning
  • Able to work independently with minimal supervision
  • Able to work well under pressure, maintains composure, multi-task, establish priorities, meet deadlines and follow through on assigned tasks
