Job purpose


You are responsible to support the Dialysis Centre to deliver holistic care by assisting in all aspects of the management to achieve maximum efficiency and cost effectiveness in the facility while maintaining a safe environment that is supportive of optimal patient care. Act as primary nurse to assigned group of patients.


Administrative Responsibilities

  • Record and update all the patient’s relevant medical history and related data and ensures to maintain confidentiality of information
  • Participate in cost control initiatives
  • Always maintain the facility in a clean, safe and sanitary environment
  • Assume other additional duties and responsibilities assigned by Nurse Manager

Staff Related Responsibilities

  • Ensure all equipment and consumables meet standard requirement, at optimal working condition safe for patient’s use and with enough available stocks to use.
  • Report the incident (relating to clinical incidents, technical issue, patients complain, compliance issue etc.) timely.
  • Join and may assign to lead the meeting of morning to pm staff for proper handover
  • Support the team for any quality improvement projects as assigned by the Nurse Manager

Clinical Duties

  • Ensure care delivery to patients in accordance to Key Performance Indicators.
  • Demonstrate competency in dialysis-related procedures including the care and procedures of dialysis access such as cannulation of arterio-venous fistula (AVF) and arterio-venous graft (AVG) and preparation of central venous catheter (CVC) based on prescription and according to the expected standards of practice and guidelines.
  • Preparation, commencement and completion of dialysis treatment, management of dialysis complications, responsible to clear dialyzers and bloodlines after dialysis, etc.
  • Responsible in assessing and documenting patient’s parameters and psycho-physio-social needs before, during and after treatment. Apply knowledge of altered health status in the assessment process. Report any abnormalities to Nurse Manager timely, inform doctor and initiate referrals to hospitals and community resources when necessary.
  • Prepare and administer medications according to protocol. Demonstrate knowledge of action, dosage and side effect of medications.
  • Demonstrate competency in collection of patient blood sample, analyse blood results, update the doctor and counsel patient as needed. Timely reporting to Nurse Manager for any abnormalities and inform the doctor if necessary.
  • Demonstrate competency in dialysis equipment related procedures according to protocol including troubleshooting, disinfection, care and maintenance.
  • Demonstrate competency in general nursing procedures e.g., dressing, resuscitation procedure, blood glucose monitoring, checking and replenishing of emergency trolley and equipment.
  • Act as the primary nurse that helps to provide the treatment plan for assigned patients. Implement treatment plan, monitor progress and review outcome regularly. Ensure patient records which includes prescription, medication, medical history, recent hospitalization, clinical data, appointment, etc. are updated.
  • Provide doctor in charge with up-to-date information of patient’s clinical data and finding for effective and appropriate intervention and prescription. This includes follow doctor consultation round and carry out their orders.
  • Refer patient to internal or external doctor if needed.
  • Assist to ensure the patients schedules are updated
  • Ensure patients and their families are educated on renal dialysis care by regular communications through Business WhatsApp account.
  • Ensure accurate, complete and timely documentation of patients’ clinical notes which includes care provided.
  • Provide update blood test results on a timely basis to ensure accurate and timely reporting of RenalTeam’s Clinical Outcome and Quality Measures.
  • Participate in quality improvement projects to pursue quality outcomes for patients.
  • Participate in plan home visit for new patients to better understand their condition and to provide the holistic care requires
  • Adhere to proper aseptic techniques and infection prevention and control protocol.
  • Demonstrate competency in collection of water and dialysate sample to send for required quality tests. Follow up the results.
  • Coordinate to external care provider for continuous patient care e.g., getting medical appointment, assist to order medications on online platform, referring to social worker, etc.
