- To conduct training and onsite support to the community care provider on the use of standardized care assessment for care planning and other purpose as per direction from Ministry of Health (MOH)
- Conduct training on the use of the appropriate InterRAI assessment tool
- Establish a working partnership with health care professionals across the care continuum to ensure that training is relevant and efficient
- Provide care assessment expertise to AIC for advisory, training and audit purposes, and be involved in the appeal process when appropriate.Effectively plan training needs for the interRAI care assessments suites for both internal and external partners
- Ensure care assessors are competent with care assessment and pre- determined the various methodology to ensure that care assessors are competent.
- Develop training and competency framework and review as needed
- Provide onsite support to the community care service providers during the implementation of the national care assessment. This involves helping the agencies onsite to conduct assessment and training, building capabilities and peer support the implementation process covering few sites at any time
- Provide support for onsite pilot studies in the conceptualization of programs in determining the appropriate care assessment tool for the sector
- Support the audit and accreditation framework and strategies to ensure quality assurance of assessments
- Contribute to the development of quality indicators working with other divisions in AIC and the community care sector
- Development of e learning system as enablers essential to the development of care assessment training platforms
Job Requirements
- Recognised Professional Degree or licensed in Nursing/Physiotherapy/Social work
- Minimum 5 years of work experience in related field
- Knowledge of healthcare sector
- Understand the various care assessment tools including interRAI
- Proficiency in service development and project management
- Ability to use local languages and dialects is an advantage
- Computer literacy with proficiency in MS Office applications
