Case Executive/ Social Worker (Residential Services)

We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will be committed to helping the Home's children navigate social and emotional challenges, providing them with the resources and support they need.


You will manage 15 cases (CANS 1 - Low Needs & CANS 2 - Moderate Needs), providing psychological, emotional, cognitive, physical, and appropriate disciplinary support for the residents residing in the dormitory. Using a trauma-informed care approach, you must endeavor to promote the resident’s best interest at all times.


Key Responsibilities:

1) Resident Management:

a) Support the Youth Work Associate in managing one of the dormitory units.

- Participate in regular circle times with all the residents in your unit to give instructions, share information, and gather feedback.

- Manage the residents in the dormitory according to their individualized care plans.

- Oversee the educational needs of your residents by attending parent-teacher meetings, ensuring school attendance, homework completion, exam preparation, and providing additional tuition support in cooperation with the Education Department.

b) Ensure the residents' safety, health, and good behavior both in the Home and during outings and appointments.

- Monitor residents' presence in the Home, unit, or supervised activities and manage permissions for leaving these areas.

- Ensure adherence to the Home's structure and routines.

- Monitor residents to prevent risky behaviors or activities that may cause injury.

- Manage resident behavior by encouraging and rewarding good behavior and addressing misbehavior in strict adherence to the Home's rules and discipline procedures.


2) Case Management:

a) Provide counseling and psychological support through regular monthly meetings with residents and maintain written records of these sessions.

b) Assess residents’ overall well-being and formulate care plans to monitor their progress and development.

- Recommend appropriate interventions and disciplinary actions/programs as part of care plans.

- Monitor and record changes in residents’ behavioral performance in care plans and provide updates during meetings or as necessary.

- Develop and enhance systems to improve existing services.

- Work with the Youth Work team to ensure the implementation of care plan interventions.

c) Collaborate with the CSLMCH Education Manager and school stakeholders to oversee residents’ academic and behavioral performance.

- Respond to emails and calls from school teachers/principals.

- Attend meet-the-parents sessions.

- Apply for bursaries and financial assistance.

- Monitor academic performance and recommend follow-up programs to promote academic improvements.

d) Work with various stakeholders and community partners (e.g., MSF & FSC) to plan and work towards family reunification.

- Maintain positive relationships with stakeholders and community partners.

- Conduct home visits when necessary, especially during home leave.

- Update residents’ families on their performance and propose reintegration when applicable.

- Conduct and facilitate programs for residents’ reintegration into their families based on approved care plans.

- Attend meetings including case conferences, case reviews, school meetings, and hospital meetings.

e) Maintain proper records of all services rendered to residents in accordance with organizational policy and MSF licensing requirements.


3) Others:

a) Perform additional administrative tasks as assigned.

b) Take on any other duties assigned by the supervisor.


Requirements:

  • A recognized Basic/Honours Degree in Social Work, Counselling or Psychology
  • Proficiency in English and a second language (Chinese, Tamil, Hindi, or Malay) is highly desirable.
  • Strong knowledge and competency in case management support, trauma-informed care frameworks, and intervention strategies in residential settings.
  • Excellent report-writing skills and the ability to meet deadlines.
  • Flexibility in work hours and adaptability to last-minute schedule changes.
  • Demonstrated passion for working with children, patience, and a commitment to making a difference in the lives of disadvantaged children and young persons.
  • Ability to work independently with a pleasant disposition and maintain good working relationships with other staff.
  • Good interpersonal skills to provide psychological support for children.
  • Strong morals, ethics, and a commitment to child welfare.
