Job Responsibilities

  • Care for the well-being and safety of clients with dementia in Kebun Baru.
  • Provide basic case management for clients with dementia or cognitive impairment.
  • Monitor and assist clients with daily living activities to promote their independence.
  • Plan, organise, overseeing and lead activities for the day-to-day execution of programmes, either one-to-one or on a group basis, depending on the preferences, needs and limitations of each client.
  • Organise and oversee social and recreational group excursions for clients.
  • Manage and accompany clients for important medical appointments and activities.
  • Monitor and organise outreach events such as talks and webinars.
  • Explore new avenues of collaboration with community partners.
  • Engage with various stakeholders to ensure smooth running of programmes.
  • Conduct the pre-admission assessment.
  • Ensure proper documentation of client care plans, records, activity and participation log.
  • Provide clients with information about available social support services from other organizations.
  • Identify and suggest enhancements for programs, activities, and training sessions.
  • Contribute to operations review meetings and offer insights for developing and revising care plans.
  • Support caregivers and volunteers in caring for clients.
  • Plan and manage the volunteer roster.
  • Assist in managing all aspects of daily activities to ensure smooth functioning.
  • Procure consumables and supplies as needed.
  • Any other duties as & when assigned.


Requirements

  • Diploma and equivalent or above.
  • Possess a passionate disposition to care for persons living with dementia.
  • Be able to engage and communicate comfortably with persons living with dementia or mild cognitive impairment.
  • Leadership skills and a collaborative team player.
  • Able to work around unexpected changes and situations.
  • Understand and able to apply the whole Person-Centred Care approach concept in the everyday running of programmes.
  • Competent to use Word and Excel.
