Conduct community outreach and relevant need assessments to the elderly at a designated
service boundary.
Conduct care befriending services to frail and vulnerable elderly at their homes.
Provide referral to help services for the elderly in needs.
Recruit volunteer befrienders to assist with the outreach and befriending services.
Train volunteer befrienders to provide monitoring and appropriate referral to support
services.
Perform administrative and reporting duties such as visitation notes, records and other
necessary data and statistics required by the authorities.
Perform other duties as assigned by the reporting officer.
Requirements:
- At least 2 years of experience in elderly befriending service / social community care sector
- Immediate starter is preferred
