We are looking for a Health Services Manager to work under the supervision of the Managing Director or designee in providing comprehensive assessment and improvement to our clients' health and wellness.To be successful as a Health Services Manager, you should have an optimistic disposition, good conflict resolution skills, and possess the ability to effectively communicate.
Domains of Practice:
Competency with Practice Parameters and Clinical Guidelines for Chronic Illnesses Management
- Domains of practice include all of the elements of the Health Coach role.
- Demonstrate familiarity with pathophysiology, epidemiology, and clinical guidelines for chronic illnesses and treatment options.
- Evaluation/Management of Chronic Illnesses
- Competent with specific clinical assessment using standardized diagnostic tools
- Monitor patient adherence to prescribed health plans.
- Collect, analyze and integrate patient information.
- Coordinate out of centre diagnostic testing and therapeutic modalities.
- Implement and follow-up on physician orders.
- Communicate effectively and professionally with the clients’ healthcare team regarding new or continued symptoms, health plans’ concerns or management of optimal care.
- Develop programs and provide education to assist clients with health plans compliance including how to track health improvement indicators.
Discuss equipment/supply replacement schedule. - Comprehensive Understanding of Teaching and Motivational Skills
Apply principles of motivational teaching to facilitate patient self-management. - Use a variety of communication and teaching techniques to optimize client education and motivate self-management.
- Encourage family participation with clients’ health plan.
- Promote community education on lifestyle medicine.
Program and Business Management
- Maintain accurate, pertinent, accessible, confidential and secure client records in accordance with privacy and security standards.
- Ensure client records contain documentation of all client interactions.
- Manage client outcomes database.
- Provide oversight of other health coaches.Develop and implement quality assurance/improvement activities and staff training.
- Participate in professional development activities.
- Increasing company’s caseload through long-term relationship building with client organisation, referral sources and targeted community.
Service Management and Essential Skills
- Maintain proficiency in the domains of practice of the Health Coach.
Comply with professional standards of conduct. - Maintain current CPR or BLS certification.
- Demonstrate proficient use of medical terminology.
- Demonstrated advanced knowledge of the pathophysiology of metabolic syndrome and associated comorbidities.
- Demonstrate effective client assessment techniques.
- Perform database management.
- Demonstrate computer skills appropriate for the position.
- Demonstrate effective written and verbal communication skills; age-specific competencies.
- Demonstrate appropriate social skills; cultural competency.
- Demonstrate customer service skills; cultural competency.
- Demonstrate critical thinking and ability to work collaboratively with other healthcare providers.
- Develop and implement policies to maintain the privacy and security of client information.
- Adhere to company policies related to quality assurance, clinical procedures and emergency preparedness and safety.
- Demonstrate ability to manage health coaches.
- Assist the Directors in all aspects of company operations.
Education and/or Experience
- Successful completion of health coach program / nursing diploma or degree / biomedical science diploma or degree with a minimum of three years experience in the health and wellness profession.
