Job Description
Home Visit Responsibilities for Chronic Ill/Palliative Patients:
- Conduct nursing assessments.
- Coordinate with doctors for medical evaluations and follow-ups.
- Monitor medication adherence.
- Perform nursing procedures (wound care, catheter insertion, etc.).
- Initiate and implement nursing care plans.
- Educate caregivers and families on basic nursing procedures for ongoing care.
Requirements
- Min. Diploma / Degree in Nursing
- Min 2 years of ward / A&E / ICU / Community / Geiatric experience
