Youth Connect Associate

Kk Women's And Children's Hospital Pte. Ltd.

We are seeking highly motivated and compassionate Social Work Coordinators to join our pioneer team for the Youth Connect Programme. You will play a pivotal role in KKH, in providing holistic psychosocial care for patients, in tandem with medical care. You will bridge the service gap between healthcare and community care settings. As a member of our dynamic and dedicated team, you will have the opportunity to work directly with specialist doctors, nurses, psychologists and social workers in healthcare teams, schools and social service agencies in the community to support the overall well-being and development of young people.

Your main responsibilities are to:

  • Engage adolescent patients and caregivers in KKH.
  • Conduct psychosocial assessments and formulate care plans collaboratively with adolescent patients, their caregivers, their healthcare providers and psychosocial support services in the community.
  • Harness relevant self-help resources for effective psychoeducation and psychosocial support for patients and caregivers.
  • Facilitate closed-looped care coordination between relevant agencies and hospital services to ensure that the youth’s psychosocial and medical needs are concurrently met.

We are looking for candidates with:

  • Higher NITEC / NITEC / GCE ‘O’ / ’N’ / ‘A’ Level with 8 years of relevant working experience, e.g. in education, counselling, pastoral care, social services.
  • Diploma in Social Work from NYP / WSQ Diploma in Social Service from (SSI) / SSI Higher Diploma in Social Service would be an advantage.
  • A strong sense of purpose in making a positive impact on the lives of young people.
  • An aptitude for engaging and building rapport with youths.
  • Good interpersonal skills to engage and communicate effectively with youths, caregivers, medical teams, community partners etc.
  • Ability to multitask in a fast-paced environment to manage concurrent tasks and deadlines.
  • A highly driven, inquiring and resourceful mindset, that relishes the challenge of pioneering work and problem solving in a ground-breaking programme.
  • A disposition that is resilient, independent, trustworthy and mature, yet empathetic, non-judgmental, and youthful at heart.
  • A humble learning attitude that values the opportunities to participate in training and professional development.
  • Possess a minimum proficiency in Microsoft Office applications.
