- Coordinate and Schedule Diabetic Foot Screening Services, Diabetic Retinal Photography and Nurse Counselling Services
- Assist in implementation and educating patients on Healthier SG and benefits.
- To work closely with the GPs to manage identified patients who need support and referral to community services and ensure that their changing needs are addressed
- Guide patients in achieving their target set as well as follow up on patient's progress where necessary. Escalate to GP when neccessary.
- To Track, Monitor and follow up on the quality of care provided by GPs and other Primary care professionals and to ensure the maintenance of good clinical care.
- Assist GPs in the Submission of data and other relevant administrative work
- Involve in the PCN planning team to ensure smooth coordination within PCN
- Perform any other task assigned by PCN leader or Supervisor
