Job Description:
- Collaborate with the care management team to coordinate care for patients.
- Conduct basic preventive health screenings and health assessments.
- Assist the clinical outcomes team with patient surveys and database maintenance.
- Monitor participant outcomes, track follow-up visits, and ensure timely data collection.
- Prepare and present monthly reports on patient outcomes.
- Attend relevant lectures, courses, and seminars for professional development.
- Undertake additional responsibilities as assigned by the Reporting Officer.
Job Requirement:
- Diploma in Science or equivalent
- With relevant experience are advantageous
- IT Savvy
