RESPONSIBILITIES
- Identify and develop training roadmaps for the PCN Care Teams (which includes Nurses, Primary Care Co-ordinators, Clinic Assistants, GPs)
- Anchor the development of standardised care delivery
- Involved in workgroups to support primary care development and transformation.
- Co-create new models of care to support team-based care approach.
- Gather insights for development and implementation of new initiatives and support the administration of the existing primary care model
- Provide clinical supervision and implement framework to ensure patient safety and staff safety for care delivered
- Consolidate and share best clinical practices across the primary care landscape
JOB REQUIREMENTS:
- Bachelor degree Nursing and / or Master degree in any discipline
- Prior experience in healthcare management or operations in primary care or hospital is desirable
- Possess a high level of adaptability and learning agility in a dynamic environment
