Job summary
The Senior/Social Worker is responsible for providing comprehensive support to clients transitioning from Home to independent living. The role involves ensuring a smooth transition and providing ongoing guidance and assistance to empower youths in various aspects of life.
Key Responsibilities:
Case Management
- Collaborate with Case Executives from the residential services to identify clients needing post-care transitional support.
- Assess referrals, coordinate triage with professionals, and determine suitability for aftercare services.
- Develop personalized transition plans with clients, addressing their needs, strengths, and goals.
- Engage with community stakeholders to address systemic barriers to reunification.
- Implement, monitor, and update case plans and interventions.
- Provide ongoing support for independent living, including educational, employment, and life skills support.
- Regularly report client progress to inform case plan decision-making.
- Support reunification efforts for clients and families.
Community Engagement
- Build and maintain a robust network of community partners and governmental agencies.
- Mobilize resources to meet client needs during and beyond the transition period.
- Coordinate accommodation options for clients lacking housing stability.
Administrative
- Ensure timely and accurate documentation and submission of work.
- Consolidate and verify monthly statistical data.
Other Duties
- Attend staff meetings, supervision sessions, and training courses.
- Participate in special projects, programs, and committees within CSLMCH.
- Supervise Social Worker Associates.
- Perform additional duties as required by the position or requested by the Supervisor.
Academic &Trades Qualifications
Essential:
- Bachelor's degree in Social Work or related social sciences (Psychology & Counseling)
- Minimum 2 years of relevant experience in children, youth, or family
Desirable:
- Master’s degree in Social Work or related social sciences (Psychology & Counseling)

Essential:
- Demonstrated understanding of reunification work, trauma, attachment theories, and child development
- Positive attitude and innovative problem-solving skills
- Ability to manage stress and crises effectively
- Openness to feedback and commitment to personal and professional growth
- Strong communication and interpersonal skills
Desirable:
- Bilingual proficiency (English and Mother Tongue/Dialects)
- Experience working with children, youth, or families
- Supervisory or leadership experience
- Ability to work flexibly and adapt to changing circumstances
