Job Summary:
The incumbent is part of TOUCH Special Needs Group (TSNG) who reports to the Centre Manager of TOUCH Ubi Hostel (TUH) / Community Support Service (CSS) and is responsible for the role of Social Worker I that encompasses implementing holistic services and programmes that will benefit and make an impact to the needs of clients under TSNG and CSS. The incumbent is responsible for delivery of quality service, personal efficiency and teamwork at the workplace
Main Responsibilities:
- Planning and executing community outreach programme and/ or support group for clients in CSS. Conduct feedback and outcome-based evaluation for programme.
- Mobilise resources and work with community partners to support the execution of community programme.
- Lead and mentor volunteers in planning and execution of CSS activities.
- Outreach to community partners to advocate for Persons with disability (PwD) and recruitment of potential CSS clients.
- Handle enquiries pertaining to programme and to assess potential clients for intake to CSS or other services under TSNG.
- Casework management for CSS clients who need support such as social-emotional support, long term care planning etc.
- Provide service/ care coordination of PwDs and caregivers from information gathering, intake assessment and case coordination with other stakeholders/agencies
- Outreach to PwDs in the community through various means such as door-to-door or roadshows etc.
- Build and maintain strong relationship with families and caregivers and organize and conduct caregivers’ support activities under TSNG.
- Work with the team in the transition planning for trainees graduating from TSNG’s services into the community.
- Work with staff from various services for special project on ad-hoc basis.
- Maintain and update membership application and renewal under CSS
- Produce programme/ event reports for documentation and submission to funders.
- Produce social report and case notes under case management.
- Maintain proper case note recordings and documentation.
- Draft articles on clients for newsletters or publicity when needed.

- 5 day work week: Mon-Fri (Bi-weekly 1 day on weekend)

Essential Skills and Qualifications:
- Degree holder or Graduate Diploma in Social Work.
- Registered Social Worker under Singapore Association of Social Work.
- Preferably with 1-2 years of social work experience in working with person with special needs.
- Proficiency in Microsoft Office applications.
- Passionate and dedicated to work with the special needs community.
