- Assist in administrative work in the delivery of advisory services to patients and their families.
- Conduct assessment / home visits when necessary.
- Liaise with relevant agencies / authority / patients family.
- Assure that all referrals are attended to by relevant personnel.
- Liaise with nurses on patient-related matters.
- Assure that all referrals are attended to by self or relevant personnel.
- Act as a channel of communication between management and the customer care coordinator.
- Handle difficult customer and communicate escalated issues to the Manager.
- Keep update records of patients ( electronic or written data)
- Up-to-date the relevant information of government’s policy with regards to the elderly care Services and funding.
