Job Description:
* Provide social casework management, conducting psychosocial and financial assessment, counselling and recommending appropriate intervention in the management of patient care
* Facilitate discharge care planning
* Participate in multi-disciplinary team meetings and facilitate family conferences
* Network with relevant organisations, social service agencies and individuals to activate social and financial resources to meet patient’s needs
* Conduct home-visits
* Organise and run psycho-emotional-social group activities for patients
Job Requirements: Min Degree in Social Work w min 5 yrs of relevant experience in medical/geriatric setting
