Job Responsibilities:
- Coordinate and Schedule Diabetic Foot Screening Services, Diabetic Retinal Photography and Nurse Counselling Services
- Assist in implementation and educating patients on Healthier SG and benefits
- To work closely with the GPs to manage identified patients who need support and referral to community services
- Tracks, Monitors and follows up on the quality of care provided by GPs and other Primary care professionals and to ensure maintenance of good clinical care
- Assists GPs in extraction and submission of data and other relevant administrative work
- Involve in the PCN planning team to ensure smooth coordination within PCN
- Has experience conducting screening and provide comprehensive health education to patients with chronic conditions
- Ability to assess patient’s medication compliance, social history, current lifestyle and dietary habits
- Has the ability to provide good clinical judgement, provide patient care in line with work instructions and protocols
- Assist in data collection for quality improvement projects
- Willing to work on Saturdays when required
- Perform any other task assigned by PCN leader or Supervisor
Job Requirements:
- Diploma or degree in Nursing
- Registered Nurse with the Singapore Nursing Board
- Experience in mental health and chronic disease management will be an advantage
- Experience in healthcare or social services
- Good interpersonal and communication skills
- Has a positive attitude and a good team player
- Fluent in English, Chinese and a local dialect to communication with senior citizens
