Job Description & Requirements
- Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members.
- Perform referrals to other service providers and conducts home visit where necessary.
- Work closely with multi-disciplinary team members.
- Support projects and programmes.
- Attend training and case work supervision.
Job Requirements:
- Degree in Social Work or a Graduate Diploma in Social Work.
- Registered with Singapore Association of Social Work.
- Experience in means testing and healthcare setting is advantageous .
- Salary up to $6000
- Working location: Toa Payoh
- Working hours: Monday to Thursday: 8.00am to 5.30pm/Friday: 8.00am to 5.00pm
