- Evaluate and Assess specific care requirements of individual elderly and develop care plans tailored to their needs.
- Create and execute programs for elderly utilisiing data analysis to monitor the effectiveness and adjust according to the needs.
- Collaborate with various stakeholders to coordinate care, facilitate service referrals, activitly participate in case discussions to ensure comprehensive support.
- Implement and evaluate new activities, review and initiate area for enhancement.
- Mentor volunteers, manage budgets and resources, and collaborate with other organizations to deliver programs effectively.
Job Requirements :
- Diploma/Bachelor’s Degree in Social Work.
- Preferably relevant experience in Social Service sector, working with elderly clients or any related field.
- Proficient in Microsoft Office (MS Word, Excel and PowerPoint).
- Able to work independently and in a multidisciplinary team to meet deadlines.
- Good interpersonal and communication (oral and written skills).
- Possess some knowledge of programme design, implementation and evaluation.
- Willing to work Mon to Fri 9am - 6pm.
