Role Overview: As a Medical Social Worker at Ihsan Kidney Care, you will be responsible for delivering comprehensive care to patients in both the dialysis center and home dialysis settings. You will conduct assessments using tools such as the Bio-Psychological-Spiritual (BPSS) assessment and the Risk & Needs Matrix to guide patient management effectively. Your expertise in medical aspects and community dialysis settings will enable you to provide vital support to patients and their families.


Job Responsibilities:

  • Conduct Dialysis Treatments: Independently administer and manage dialysis treatments for patients, ensuring adherence to established protocols and safety standards.
  • Patient Monitoring: Continuously monitor patients during dialysis sessions, closely observing vital signs, fluid levels, and overall well-being to promptly address any complications or changes in their condition.
  • Emergency Response: Be prepared to respond promptly and effectively to any unforeseen emergencies that may occur during dialysis procedures, such as adverse reactions or medical crises.
  • Documentation: Maintain accurate and up-to-date records of patient information, treatment plans, and progress notes in compliance with healthcare regulations and facility policies.
  • Patient Education: Educate patients and their families about dialysis procedures, treatment options, and self-care techniques, ensuring they have a comprehensive understanding of their condition.
  • Medication Management: Administer prescribed medications and intravenous therapies as required, ensuring proper dosage and patient safety.
  • Collaboration: Collaborate with the healthcare team, including physicians, technicians, and support staff, to provide comprehensive care and treatment plans for patients.
  • Infection Control: Follow strict infection control protocols to maintain a sterile and safe environment in the dialysis center, including proper cleaning and disinfection procedures.
  • Quality Assurance: Participate in quality assurance programs and adhere to regulatory standards to maintain the highest level of patient care and safety.
  • Assist with Administrative Tasks: Provide support for administrative tasks within the center, including patient scheduling, inventory management, and maintaining necessary records and reports.

Job Requirements:

  • Candidate must possess at least a Diploma, Advanced/Higher/Graduate Diploma, Bachelor's Degree, Post Graduate Diploma, Professional Degree in Nursing or equivalent
  • At least 2 years (s of experience in renal nursing or a related field is preferred.
  • To be registered with the Singapore Nursing Board and have the relevant nursing certificates
  • Able to work in different shifts (Eg Morning, Afternoon, Evening)
  • Strong clinical skills and the ability to work efficiently under pressure.
  • Excellent communication and interpersonal skills.
  • Compassion and empathy in providing patient care.
  • Knowledge of dialysis equipment and procedures.
  • Ability to adapt to changing healthcare protocols and technologies.
