Community Outreach Executive

Location: Senja Care Home


Responsibilities:

  • Support the Manager in overseeing compliance in day-to-day operations, directives, KPI and all work processes.
  • Conduct home visits, community outreach efforts and conduct community screening for seniors residing in the assigned boundary.
  • Develop and implement outreach strategies.
  • Build and maintain relationship/ rapport with community seniors.
  • Plan and implement activities and programmes based on seniors’ needs and interest to promote active lifestyles and social engagement.
  • Gather seniors to join the planned programmes.
  • Provide support to seniors who requires befriending and track the efforts done.
  • Provide information and refer the community seniors to appropriate services as required.
  • Recruit, develop and retain volunteers/ senior volunteers.
  • Assist community seniors on vital signs monitoring.
  • Compile and submit required reports on a timely basis.
  • Provide administrative support such as putting up document for purchases, claims and processing invoice received.
  • Track the expenses for the AAC and keep expenses within budget.
  • Supervisory role to associates and ensure their competencies.
  • Arrange and track essential courses required.
  • Any other duties as assigned by the Manager.
  • Attend to walk-in, phone and text enquiries and correspondence.
  • Track AAC KPIs such as outreach, programmes participation and befriending.
  • Ensure confidentiality of the seniors’ data, in line with Organisation’s PDPA policy

Requirements:

  • A degree in any discipline.
  • 3 to 4 years' experience in community care setting is preferred.
  • Good communication and interpersonal skills.
  • Can-do positive attitude, with interest to engage the seniors.
  • Team player who is organized and meticulous at work.
  • Eye for details and meticulous in planning of events from start to the end
  • Willing to work on occasional weekends.
  • Adaptable to changes
  • Ability to multi-task and perform under pressure.
  • Able to work in a fast-paced environment.
  • Proficient in Microsoft Office and data presentation
