Reporting to Director, Operations, RHS Office, the incumbent will Iead in developing healthy precincts and building community partnership to keep the community healthy.
Healthy Precinct is a geographical location with an environment and culture that supports residents to achieve and sustain healthy lifestyles.
Job Responsibilities:
- Lead in conceptualising the overall strategy and framework for Healthy Precinct.
- Plan and operationalise Health Precincts to achieve health-social integration with internal and community stakeholders.
- Develop and track progress of workplans and workstreams on Healthy Precincts.
- Develop and operationalise plan to empower community to take care of their health.
- Create community culture to drive behavior towards leading a healthy lifestyle and emphasis on preventive care with community stakeholders.
- Build partnership with internal and community stakeholders e.g. grassroots organisations, social service agencies and healthcare providers to keep patients and residents healthy and integrated care across care settings.
- Develop and implement initiatives for integrated care and population health improvement.
- Lead and support the team
- Ensure department is appropriately resourced to carry out its mission - people, funding, skill & technology
- Develop team members to enhance their skills & harness their potential
- Integrate team with the other RHSO departments & divisions
Requirements:
- Bachelor Degree in healthcare or business related fields
- At least 10 years of relevant experience in healthcare, corporate/strategic planning and partnership development, including minimum 5 years of leadership experience
- Proven strategic thinking and planning ability
- Excellent writing skills
- Good interpersonal and communication skill
