Responsibilities:
- Handling social work matters to ensure social care needs for seniors and family members are met holistically.
- Supports planning and implementation of programmes
- Provide psychosocial assessment, intervention and case management to clients
- Develop and implement new community development initiatives
- Network with community stakeholders such as Agency for Integrated Care, Social Service Organisations and Restructured and Community hospitals for effective care integration
- Ensure smooth execution of the Caregiver Training Programme including administering the curriculum and training schedules.
Requirements:
- Degree in Social Work
- Registered with Singapore Association of Social Work
- At least 5 years of related experience
