Responsibilities:
- Develop individualise care plans for residents
- Execute the care plan and monitor progress
- Coordinate care for residents with relevant stakeholder
Requirements:
- With a minimum of Degree or Graduate Diploma in Social Work.
- Registered Social Worker (RSW) with the Singapore Association of Social Workers is advantageous.
- Fresh graduates are welcome to apply.
