As the Head of Community Care at Loving Heart, you will lead and oversee the strategic direction, programme development, and service delivery for community care initiatives.
This includes driving comprehensive case monitoring, building and managing stakeholder relationships to create a strong referral network, and developing impactful programmes for vulnerable individuals. Reporting to senior leadership, you will play a pivotal role in aligning services with Loving Heart’s mission and community care goals.
Responsibilities:
1. Strategic Case Monitoring
- Lead and refine case monitoring processes to ensure vulnerable individuals receive holistic, tailored support based on their unique needs.
- Oversee the development and review of personalised care plans, coordinating interventions that address social, health, and lifestyle needs.
2. Stakeholder Engagement & Partnership Development
- Build and sustain productive relationships with key stakeholders, including healthcare providers, social services, and community partners, to create a comprehensive referral network.
- Engage with stakeholders to streamline service delivery and increase support for vulnerable community members.
- Act as a key representative of Loving Heart in stakeholder meetings and community outreach efforts, enhancing visibility and strengthening partnerships.
3. Programme Development & Implementation
- Direct the design, implementation, and evaluation of programmes to meet the needs of vulnerable community members, ensuring alignment with the organisation’s values and strategic goals.
- Guide community outreach initiatives and identify new programme opportunities based on evolving community needs.
- Oversee needs assessments to ensure programme relevance and impact.
4. Leadership & Team Development
- Lead and mentor the Community Care team, fostering a culture of learning and growth that builds capacity and enhances team skills.
- Ensure comprehensive documentation and reporting on programme activities, case monitoring, and outcomes to meet internal and funder requirements.
5. Other Duties
- Perform any other responsibilities as assigned by reporting officer.
Qualifications & Requirements:
- Advanced qualification in Social Work, Community Care, Gerontology, or a related field
- 4+ years of experience in community care, social services, or a related sector, with a focus on vulnerable populations
- Demonstrated experience in partnership and stakeholder management
- Strong programme development and evaluation skills
- Excellent communication, leadership, and interpersonal skills
- Proficiency in data management and Microsoft Office tools
