As a Care Manager, you will be responsible for the following duties:
- Identify and assisting patients who require care management plan
- Monitor residents’ daily schedule and care plan.
- Monitor and tracking patient's progression to ensure optimal clinical / non-clinical outcome
- Engage with residents and family members to determine satisfaction with quality of care
- Work with clinical team and stakeholders to provide quality care to the resident
- Documenting and ensure confidentiality of patient's records
- Participate in quality improvement projects
- Perform / participate in outreach programs or activities
- Any other duties required within the team
Requirements:
- At least Diploma / Degree in Nursing and a post basic Advance diploma in Nursing (Gerontology) or Chronic Disease Management
- Possess 3-5 years of chronic disease / diabetes management
- Must have valid Singapore Nursing Board license
