We invite caring and empathetic Nurse Counsellors to join our PCN team. As a Nurse Counsellor, you play a vital role in the delivery of healthcare in the Primary Care Network (PCN). You will provide essential support to patients with chronic diseases and help them to manage their conditions effectively. By working with patients to develop self-management plans and providing on-going support, you can help to improve the quality of life for patients and reduce the chronic disease burden on the healthcare system.
Responsibilities
Patient Care
- Provide in-house ancillary services such as diabetic retinal photography, diabetic foot screening, counselling, spirometry
- Empower patients in self-management of their health and chronic conditions by setting & achieving personalised health goals, identify patient barriers to goal achievement and developing strategies to overcome identified gaps in their health plan
- Propose to clinic doctors suitable recommendations and referrals to appropriate healthcare services corresponding to patient assessment outcomes
Patient Education & Health Promotion
- Educate patients and caregivers on the proper administration of medications and development/ progression of chronic diseases
- Provide personalised healthcare recommendations to patients based on assessment outcomes
- Empower patients with tools and resources to enhance self-management skills
Communication & Team-Based Care
- Integrate motivational interviewing and behavioural change strategies to empower patients for self-management
- Establish rapport with clinic doctors and teams through open communication and reliable follow-through on commitments
- Participate in interdisciplinary team meetings and collaborate with healthcare partners, e.g., GPs, healthcare institutions, service providers, on patient care
Professional & Leadership Development
- Adhere to ethical clinical guidelines, maintain strict confidentiality and uphold professional clinical standards in engagement with all PCN stakeholders
- Stay abreast of developments in healthcare polices, counselling techniques and mental health research
- Promote a positive team learning culture and adopt best practices whilst streamlining workflows with all PCN stakeholders
- Contribute to quality improvement projects (QIPs) and discussions with clinic doctors and the PCN team
- Support the implementation of the Healthier Singapore (Healthier SG) initiative, which aims to help Singaporeans and Permanent Residents take steps towards better health through a choice of their preferred family clinic, a customised health plan and access to community programmes
Qualifications
To be a PCN Nurse Counsellor, you should possess the following requirements:
- Registered with the Singapore Nursing Board (SNB) with a keen interest in community health
- At least 2-5 years of experience working in a healthcare institution
- Possess the following traits: a positive attitude, a caring heart, a passion for helping others, the willingness to learn and grow, the willingness to listen and consider new ideas, opinions, suggestions
Job Summary
- Provide ancillary services, i.e., diabetic eye screening, diabetic foot screening, nurse counselling, spirometry, in our network of clinics during office hours on weekdays
- Build rapport with clinic teams to better facilitate ancillary services
- Complete core training provided by in-house and external trainers through didactic lectures, workshops, group discussions, on-the-job training, self-directed learning
