Purpose of the Job
Assist the Centre Manager in implementing the eldercare project and managing the Active Ageing Centre effectively.
Key responsibility areas:
- Conduct regular individual or partnered home visits to private and HDB residents.
- Maintain accurate records of home visits and client attendance.
- Utilize screening tools to assess seniors and facilitate mind-stimulating activities.
- Ensure seniors' participation in planned programs and follow up accordingly.
- Refer seniors to appropriate services in financial and counseling domains, providing support and escalating urgent cases as needed.
- Compile and submit reports, claims, documentations, or statistics to internal and external stakeholders.
- Handle simple claims and issue invoices as required.
- Assist in administrative duties as needed.
- Undertake additional responsibilities as assigned by the Centre Manager/Management.
Required qualifications and experience
- Bachelor's degree in Social Work or a related discipline preferred.
- Previous experience in healthcare or senior care centre setting is desirable.
- Proficient in MS Applications like Excel, Words, Powerpoint.

- Self-motivated individual with excellent coordination and organization skills.
- Patient and tactful with strong interpersonal skills.
- Skilled in both written and spoken English, Chinese, and local major dialects to effectively engage with seniors.
