Job Responsibilities:
- Provide personalised, and cost-effective care to the residents in the nursing home in accordance to established policies and procedures, standards of nursing care, and practices.
- Provide a holistic, multifaceted and person-centred approach to plan and execute care to meet resident's nursing needs.
- Able to harness team's cooperation and inspire them to deliver effective, safe, and coordinated care to all.
- Reaches out proactively to residents and their families regarding discharge. Plans, trains, and coordinates a smooth transaction from a nursing home to a home for them.
- Report to Nurse Manager/Director of Nursing
Job Requirements:
- Candidate must possess at least Bachelor's Degree/Diploma in Nursing
- Possess a State Registered Nurse license
- Valid Practising License with Singapore Nursing Board
- At least 2 Year(s) of working experience in an acute / community hospital or nursing home
- Possess interpersonal skills and is passionate working with elderly.
