1. Job Functions:
- Provide case management support and counselling services to residents and their families
- Plan, review and conduct rehabilitative programmes for residents
- Prepare, maintain case files, prepare Individualised Service Plans (ISPs), progress and closure reports
- Refer residents and their families to appropriate agencies for further intervention
- Analyse trends and emerging needs of counselling and case management services
- Direct and oversee the planning, development and implementation of case management services and projects
- Develop and implement outreach and communication plans for halfway house’s services
- Provide supervision, guidance and advice to social workers and social worker aides
- Conduct regular meetings with team members regarding the service delivery and standards
- Ensure that the halfway house’s Programme meets the mission, objectives and procedures of case management services
2. Candidate Requirements
- At least a Degree in Social Work, Psychology or Sociology from recognised universities
- Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage
- Minimum 5 years of relevant social work experience in the field, preferably working with offenders, and with at least 2 years of supervisory experience
3. Working Hours
5 days work week
Monday to Friday - 9am to 6pm
