You will provide good pharmaceutical care to our patients in inpatient and outpatient settings.
You will have opportunities to run ambulatory clinics such as the Anticoagulation Clinic, Cardiology, Geriatric, Renal and Smoking Cessation Pharmacist Clinics. You may provide services such as the Antimicrobial Stewardship Programme (ASP), Inpatient Anticoagulation Consults (IPAC), Integrated Diabetes Control Programme (IDCP), and take care of critically ill patients in the Intensive Care Units (ICU).
On a daily basis, you will review patients’ prescriptions and medication charts for appropriateness of drugs prescribed and perform interventions on drug-related problems. Where applicable, you will recommend and monitor patient-specific drug therapy. You will dispense medications to patients, explaining the indications and side effects for drug therapy and instructions for administration. Where needed, you will provide drug information to other healthcare professionals.
Requirements
- Candidate must possess at least a Bachelor’s/Master’s/Doctoral Degree in Pharmacy/Pharmacology or equivalent
- At least 3 years of clinical experience
- Full registration with Singapore Pharmacy Council OR Full registration in the country of attained qualification (required to undergo 6 months pre-registration training)
- Able to work in a fast-paced environment
- Good listening, written and communication skills
