Roles and Responsibilities:
- Referral and Stakeholder Management:
Engage partners for referrals and manage relationships with stakeholders. - Patient & Caregiver Support:
Assess, create care plans, and track patient progress. Provide counseling and connect to appropriate services. - Financial Counseling:
Assist with admissions, treatment costs, and guide patients on financial options (e.g., insurance, Medisave). - Patient Visits:
Conduct home, hospital, and dialysis center visits for assessments and counseling. - Advocacy:
Protect patient welfare and rights. - Collaboration:
Work with healthcare professionals and social agencies for patient care. - Support Groups:
Organize support programs for patients and caregivers. - Process Improvement:
Review and improve care processes. - Discharge/Transfer Facilitation:
Assist with patient discharge and transfer. - Other Duties:
Perform tasks assigned by the department head.
