Job Scope
- Evaluate, plan and implement care of patients with chronic disease.
- Conduct teaching and counseling for patients and their families / caregivers.
- Coordinate patient care where required.
- Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services.
- Ensure database is relevant and up to date.
- Assist to review and develop patient education materials
- Review and evaluate work processes
- Train and guide newly joined coordinators in delivery of patient care where needed.
- Participate in quality improvement activities.
Requirement
- Bachelor's Degree in Public Health / Life Sciences or equivalent
