RESPONSIBILITIES
- Receive and assess referrals to ensure appropriate placement in Centre.
- Provide end to end casework management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment.
- Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
- Address issues related to grief, adjustment, and coping with chronic illness or disability.
- Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
- Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.
- Work closely with multidisciplinary team to ensure that holistic needs of the resident/client are actively considered in their care planning.
- Ensure "hands-on" approach to working with residents/clients through individual work, group work, counselling, or other appropriate modalities.
- Maintain accurate and current documentation of casework for assigned caseload.
- Work closely with residents, clients, and families to maintain or re-establish family relations when appropriate.
- Provide support in end-of-life care to residents, clients, and their families. This may include bereavement support to family members of the deceased and practical matters such as referral to casket company for last rite matters, after office hours.
- Conduct review of resident/client's socio-economic status and make appropriate recommendations for financial assistance if necessary.
- Participate in multidisciplinary meetings and work closely with clinical care team in discharge planning reviews.
- Attend case conferences, meetings, network with external agencies as required.
QUALIFICATIONS
- Bachelor Degree in Counselling/Social Work/Psychology or related discipline.
Requirements
- Preferably with at least 2 years' work experience in social services field (MOH or MSF) performing case management tasks.
- Experience working in a healthcare setting and knowledge of healthcare financing will be a plus.
- Able to work independently as well as be a team player.
Working Hours
Under normal circumstances the hours of duty will be as follows:
- 8.30am to 5.45pm (Monday to Friday, 45 minutes lunch break)
