COMPANY DESCRIPTION
THKMC was incorporated on 13 October 2011 as a charity to provide multiple social and welfare services to the community at large, and achieved an Institute of Public Character (IPC) status on 18 November 2011. THKMC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed and culture. THKMC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors' Community, and Therapy.
RESPONSIBILITIES
The Early Intervention Programme for Infants & Children (EIPIC) helps equip children with motor, communication, social, self-help and cognitive skills. THK EIPIC Centre involves structuring classes around small groups encompassing early childhood special education, social work and therapy support services to stimulate their adaptive functioning and school readiness skills.
Reports to the Senior Social Worker, HOD (Social Work).
Main Duties and Responsibilities:
- Case Management Support
- Facilitate parents' understanding of programme and needs of child, and enrolment of students into EIPIC.
- Establish rapport, engage parents and conduct holistic bio-psycho-social assessment of child's and family's needs, risks and strengths
- Demonstrate knowledge of available community resources, evaluate and identify areas for follow up (e.g. referral to internal/external resources)
- Documentation and administrative tasks
- Participate in team meetings/case discussion, take notes and maintain good record keeping
- Complete documentation (e.g. case recordings) accurately and promptly for other professionals who are working with child
- Maintain accurate records, prepare reports for governing agencies, collate information and submit government means-test applications
- Provide logistical and administrative support of programmes which are conducted by the centre/department.
- Any other duties assigned by the Senior social worker/HOD.
- Minimum Diploma in Social Work
- 1 to 3 years of experience working with families and children preferred, fresh graduates may apply
- Team player, meticulous and have good administrative skills
- Able to work independently
- Have some understanding of the diverse cultures and races
- Has patience and commitment in the education of and nurturing young children, empowering the families
- Keen to develop professionally
