Position: Locum Medical Social Worker
Rate of Pay: $25 to $28 per hour
Period: 6 December 2024 to 23 January 2025
Convertible to Permanent: Yes
Working Hours:
- Monday to Thursday: 8:30 AM to 5:45 PM
- Friday: 8:30 AM to 5:30 PM
- One Saturday every 4 weeks: 8:30 AM to 12:30 PM
- Total: 42 hours per week
Key Responsibilities:
- Social & Financial Assessment: Perform social and financial assessments of patients and provide appropriate recommendations.
- Counselling & Family Conferences: Offer counselling to patients and their families, and take a leading role in family conferences.
- Collaboration & Networking: Work closely with relevant organizations to ensure comprehensive care for the elderly, fostering collaboration in patient care.
- Support for Destitute Patients: Address the social needs of patients who are destitute, ensuring they receive the necessary care and support.
- Multi-Disciplinary Team Participation: Contribute to the patient-care team, ensuring social work perspectives are integrated into treatment planning and care delivery.
- Groupwork: Conduct groupwork with elderly patients or their caregivers, offering support and resources to improve their well-being.
- Support Programmes: Lead initiatives and implement support programs aimed at enhancing the care and support of elderly patients and their caregivers within the community.
Job Requirements:
- Degree and postgraduate diploma in Social Work.
- Must have an interest to work with the elderly in a healthcare setting
