We also offer other services for seniors such as active ageing centres, community support for vulnerable seniors, and a sheltered senior group home. As a key member of our Community Partnership team, you will play a vital role in empowering our Seniors of Interest to thrive in their communities. Your primary focus will be identifying, planning, and implementing comprehensive programs that support ageing-in-place, enabling seniors to live independently and confidently. Additionally, you will contribute to successfully executing our Community of Care (COC 2.0) - Go Broader project. This exciting opportunity is based at either AAC-Bukit Merah View or Jurong Central Plaza, where you will join a dedicated team making a meaningful impact in the lives of seniors.
Planning and Operations
- Develop and execute outreach plans and manage targeted active ageing programmes for Seniors of Interest
- Support Active Ageing Centres (AACs) to implement evidence-based initiatives and support outreach efforts
- Assist in evaluating care ecosystem efforts (e.g.: programmes, workflows)
- Develop publicity materials and plans with the support from Corporate Communication representatives
Partnerships Engagement
- Support the implementation of new initiatives and coordinate partner engagement efforts
- Create an asset map of community partners to identify service gaps and strengths
- Collaborate with partners and government agencies, and attend networking events as needed
Administrative Work
- Collect and analyse data, document learnings, and ensure timely report submissions
- Ensure accurate record-keeping and report submissions in a prompt and timely fashion
- Support evaluation and reporting processes for management and funders
Any other duties as assigned by the reporting officer
May be required to perform additional duties and work over the weekends or after office hours.
QUALIFICATIONS- Diploma in Social Work, Community Development, Counselling or Psychology preferred
- At least 2 years experience in programme planning, care coordination, or community work
- Experience in project management, business development or relationship/stakeholder engagement will be an added advantage
