This role will provide administrative and operational support to ensure the smooth admission and discharge of clients.
Location: Tampines Care Home
Client & NOK Engagement Related Scope
- Attend team meetings to discuss and follow up on administrative care referrals for clients where necessary.
- Liaise with external organisations that provide services to meet clients’ and their families’ needs, and refer them to community support organisations if necessary.
- Assist Social Worker to follow up with clients and family members who requires financial assistance.
Administration / Operation
- Participate as part of the administration team to conduct client’s pre-admission, admission and discharge.
- Conduct intake assessment including basic financial counselling and service agreement.
- Establish and confirm source of social facts shown in the social background of all new clients.
- Support Medifund administration and applications in collaborations with Finance and Social Woker.
- Support and work closely with Finance to manage arrears.
- Provide day to day coordination roles and duties between NOKs and Centre.
- Maintain and upkeep administrative documentation of all clients including filing, tracking and updating where necessary.
- Maintain and upkeep Centre’s databases
- Manage and update data into relevant system such as IRMS, ILTC and NMTS.
- Any other duties as assigned by the Reporting Manager/Supervisor
Requirements:
- Diploma in relevant field
- Demonstrate initiatives to identify, plan, develop and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefits stakeholders.
- Good communications and people engagement skills
- Good organisation ability
- Ability to multi-task and eye for details.
Years of Experience Required:
- Candidates without experience are welcome to apply.
- At least 1 to 3 years of relevant healthcare industry experience.
