Mon to Thu, 8:30 am – 5:45 pm
Fri, 8:30 am – 5:30 pm
One Saturday/4 weeks, 8.30 am – 12.30 pm
Total of 42 hours per week
Job Scope:
- Perform social and financial assessment of patients and make recommendations.
- Provide counselling to patients and their families and lead in family conferences.
- Liaise with relevant organizations for collaboration and networking in care of the elderly.
- Attend to the social needs of destitute patients.
- Participate in a multi-disciplinary team on patient-care.
- Conduct groupwork with the elderly patients or their caregivers.
- Implement initiatives and support programs that enhances the care and support of elderly patients and their caregivers within the community.
Requirements:
- Advanced Diploma/Deg in Social Work
- Some experience in social work will be advantageous
