Job Summary:
The Chronic Kidney Disease (CKD) Care Coordinator is responsible for assessing, planning, and implementing care for patients with chronic kidney disease. This role involves patient education, coordinating care, collaborating with healthcare teams, maintaining accurate patient records, and providing ongoing updates to program leadership. The ideal candidate will ensure the delivery of high-quality care, maintain a safe working environment, and contribute to continuous improvement efforts within the CKD program.
Primary Responsibilities:
- Patient Care Management
- Evaluate, plan, and deliver care for patients with chronic kidney disease, ensuring adherence to established clinical protocols and care pathways.
- Conduct educational sessions and counseling for patients, families, and caregivers to support treatment adherence and enhance patient outcomes.
- Coordinate patient care across various service providers and healthcare teams as required.
- Monitor and manage patient progress, adjusting care plans as necessary in collaboration with the care team.
- Collaboration and Coordination
- Work closely with physicians, renal specialists, and other healthcare providers to deliver integrated and comprehensive care.
- Facilitate communication and collaboration within the multidisciplinary team to ensure continuity of care.
- Documentation and Reporting
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational standards.
- Provide regular updates and reports to the Programme Manager and/or Renal Champion Doctor, highlighting patient progress and any relevant clinical developments.
- Workplace Safety and Hygiene
- Ensure the clinic or care environment is maintained in a clean, safe, and organized manner, adhering to all health and safety protocols.
Secondary Responsibilities:
- Education and Process Improvement
- Assist in the development and review of patient education materials, collaborating with physicians and other healthcare providers as needed.
- Support the review and evaluation of clinical processes to identify opportunities for improvement in patient care and program efficiency.
- Mentorship and Training
- Provide guidance and support to newly joined coordinators, assisting with the onboarding process and patient care delivery.
- Quality Improvement
- Participate in quality improvement initiatives aimed at enhancing patient outcomes, operational efficiency, and care standards.
- Additional Responsibilities
- Take on other duties or tasks as assigned by the Reporting Officer or Programme Manager.
Job Requirements:
- Bachelor Degree in Life Science, Medical Sciences and/or related medical fields.
- Proficient in Microsoft applications.
- Team player with excellent interpersonal and communication skills.
