Job Purpose
• The case management officer reviews patients (inpatient and outpatient) and assess suitability for enrolment under Cardiac Rehabilitation Programme (CRP). These include assessing, coordinating and implementing care, teaching and counselling plans to help patients and their families with more complex needs to adapt to changes.
• The case management officer is also responsible for clinical practice and patient education in the nursing specialty / subspecialty.
Primary Responsibilities
• Identify patients through proactive screening daily.
• Assesses, plans, implements and evaluates, conducts counselling and teaching for patients and their families/caregiver.
• Provides patient/family information and education on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge.
• Ongoing collaboration among patient, family and a multidisciplinary team to provide valuable connections.
• Refer patients to various need or department accordingly (e.g. Dietician/ Physiotherapist/ Smoking Cessation Counsellor)
• Screen, coordinate selection and recruitment of patient into the outpatient Cardiac Rehabilitation Programme (CRP)
• Schedules outpatient Pre-Rehab Assessment Clinic follow-up appointments when appropriate
Outpatient CRP (Heart Rehab) – Clinical
• Referral can be from heart clinics, wards, self-referral, etc.
• Triage the referral, contact patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient’s visit to respectively physician/therapist appointment booking.
• Check and highlight clinic / CRP orientation appointment list for enrolled patients.
• Coordinate and assist in Pre-Rehab Assessment clinic and schedule Post Rehab Clinic at the end of the program with post program parameter.
• Input the information into the Rehab Clinic database
• Assume responsibility for data collection and entry into CRP database system.
• Coordinate patients’ access and treatment sequence where required for the specialty.
• Tele-care to reschedule patient’s appointment date when defaulted outpatient follow up
Outpatient CRP (Heart Rehab) – Patient Education
• Conducts nurse-led specialty education session for patients and their families.
• Plans, implements, and evaluates in-service talks and specialty training courses based on unit and individual training needs
• Ensures patient education and counselling is carried out according to established standards of care and practice using the approved teaching materials.
• Applies understanding of prevention of medico-legal issues in relation to patient education.
• Assists to review and develop patient education materials in consultation with doctors and paramedical staff.
Outpatient CRP (Heart Rehab) – Operations
• Ensure daily operations of these service points, including administrative/operational matters and partnering Clinic Manager to solve system and process problems.
• Handle purchase, work requisition processes and calling suppliers for quotation.
• Ensure sufficient supply of related educational material / stationery requisition and other equipment maintenance are maintained.
• Updates and maintains patient consultation/CRP training statistics and CRP support group.
Secondary Responsibilities
• Acts as a resource person to nurses, nurse manager / nurse clinician on matters and issues for the specialty.
• Assist and coordinate hospital educational talks and external talks / workshops / forums / seminars.
• Participates in quality improvement / research projects.
• Participates in training /quality improvement / research committees.
• Assume any additional responsibilities as assigned.
Requirements
• Min Bachelor of Nursing
• Min 2 years of clinical experience in Acute / Cardiac Nursing
