Job Description:
- Identify patients proactive screening daily
- Provide patients and their family education on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge
- Screen, coordinate recruitment of patient into the outpatient programme
- Triage the referral, conduct patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient's visit
- Updates and maintains patient consultation/ CPR training statistics and CPR support group
- Any ad hoc duties as per assigned
Requirement:
- Bachelor Degree in Nursing or Allied Health or Science related.
- Minimum 3-5 years experience
