1. Job Descriptions
- Attend to general health and care of all residents
- Assist the Social Worker in performing Resident Assessment Form (RAF) for potential residents into the HOME
- Update colleagues on residents’ medical and nursing matters
- Advise Head of Home and Deputy Head of Home on admission of potential residents and discharge of residents from medical and nursing viewpoints
- To conduct RAF for residents every six months or upon request from Head of Home
- Accompany residents to clinics and hospitals
- To update colleagues of residents’ upcoming or rescheduled medical appointments
- Required to be on ‘standby’ for emergency and crisis of residents
- To conduct and document weekly hypo count tests for residents
- Update residents’ medical records regularly
- Top up residents’ medication when necessary, and to prepare medication when residents go on ‘home leave’
- Assist in social and outdoor activities, when necessary
- To ensure that residents are medically compliant
- Support all PERTAPIS programmes and activities and perform any other duties assigned by the management as and when required
- Carry out any other tasks assigned by the Head of Home, CEO and higher management
2. Job Requirements
- Nitec in nursing or relevant qualifications
- Relevant experience in similar field
3. Work Schedule
- 6 days work week (3 rotating shift including weekends/Public Holiday
