You will be working closely with the Nurse/ Care Manager to provide safe, cost effective and quality nursing care to patients in accordance to established hospital policies and procedures, standards of nursing care, professional competencies and practices to achieve the desired patient outcomes. He/she should practice within enhanced levels of clinical competence and decision-making skills whilst being imbued with clinical reasoning and critical thinking skills that promotes safety of patient and the team members. He/she shall be a good team player, be analytical, insightful, and highly motivated.
A little about you and what we seek to accomplish together:
- Implements patient care in collaboration with the multi-disciplinary teams (MDT) and according to assessed needs and health condition(s).
- Facilitates admissions, transfers and discharges; ensures discharge planning is done timely upon patient’s admission and appropriate to condition.
- Follows up and evaluates patients’ progress according to planned care and readiness for discharge.
- Collaborates with the MDT to deliver evidence-based care to patients to achieve the desired care outcomes.
- Monitors patient for changes in condition, recognises early signs of clinical deterioration using appropriate physiological assessments and escalates as required to enable timely implementation of interventions.
- Participates and contributes actively in MDT’ rounds, executes physicians’ treatment and investigation orders promptly.
- Prepares and assists physicians with bedside patient procedures and treatments.
- Communicates and discusses patients’ progress and response to treatment in collaboration with the MDT.
- Demonstrates clinical skills, knowledge, critical thinking and reasoning in the delivery of patient care.
- Assists nursing team to problem solve if there are any patient/ clinical/or operational issues and seeks appropriate assistance/escalates to Senior Nurse Manager (SNM)/Nurse Manager (NM).
- Delivers patient and/or family education on disease progression, prevention and rehabilitation and assesses understanding of condition to enable participation in self-health care management.
- Assists department supervisors to minimise use of resources keeping expenses within approved budget.
- Reviews infection control practices, monitors compliance and identifies areas of improvement.
- Plans and implements in-services for nurses and students to extend skills and knowledge to maintain safe and quality nursing care.
- Supports new initiates, leads changes implemented by Nursing in collaboration with other stakeholders.
- Reviews clinical performance of new nurses and nursing students and provides constructive feedback and documentation to appropriate personnel.
- Acts as a competent clinical resource and supervises:
- use of medical equipment;
- use of Electronic Medical Records (EMR) and electronic systems;
- clinical workflow and processes;
- co-leading in clinical emergencies or crisis situations
What you might need to have:
- Preferably an Advanced / Diploma in Nursing and/ or Bachelor in Nursing
- Valid practicing license with Singapore Nursing Board
- At least 2 years of experience as SSN I in a clinical setting
- Ability to exercise good judgment and problem-solving skills
- Possess good grasp of clinical workflow in Community Hospital and Long-Term Care settings.
